To schedule an appointment, please contact our staff Monday through Friday, from 8 a.m. to 6 p.m. After hours, please call (425) 451-4399.
Please arrive 10-15 minutes before your appointment. If you are unable to keep your appointment, kindly give 24 hours notice. Otherwise, a charge may be made for time reserved.
If you are a new patient, please bring your insurance card with you. If you have any other information that may be helpful - such as phone numbers for your benefits representative at work, the plan's case manager for claims, the claims processor for your employer group, or other important people to contact - please bring that information as well.
Our billing staff will submit claims to your insurance company for all services you receive. Our staff will submit insurance claims daily, so you should receive reimbursements from your insurer promptly.
Deductibles, co-pays or patient portion are due at the time of service.
If this policy presents an undue hardship to you or your family now or at any time in the future, please feel free to discuss your situation with me or the billing supervisor.
All of your medical records will be held in strict confidence.

Coming south on I-405: Take the NE 8th exit and go east. Get into the left hand lane as soon as possible. Turn left at the first stop light (which is 116th). Go north on 116th to 1370 116th Avenue NE. (Overlake Hospital is on the left side of the street.) Turn right in front of the sign for the Eastview Professional Building.
