Dear Colleagues,

Today's topic is chronic illness. According to a report by Partnership for Solutions, almost half of all Americans, 133 million people, live with a chronic condition. By 2020, that number will increase to 157 million. People with chronic conditions account for 75% of our national spending on health care, and for many, the consequences of severe chronic illness are isolation, depression, and physical pain. Successful management of chronic conditions requires collaboration among patients and their various healthcare providers.
