Chronic illnesses such as heart disease, cancer, and diabetes are among the leading causes of disability and death in the U.S. According to the Centers for Disease Control, chronic diseases cause major limitations in daily living for more than 1 of every 10 Americans. Treating comorbid psychiatric conditions and helping patients improve their quality of life are significant challenges in chronic disease care. Today's resources focus on the treatment of adult patients with chronic illnesses.
